For good and valuable consideration, the receipt of which is hereby acknowledged, I hereby consent and authorize the use and release of any and all photographs, video footage, and/or stories that Powerhouse Theatrical Society or their duly authorized agents have accepted or produced of myself for the promotion, advertising and trade of that Powerhouse Theatrical Society without compensation.

I understand that these photographs, video footage and stories may be used multiple times in written publications (newspapers, brochures, reports etc.), web content and posts on social media channels.

The photographs, video footage, stories and copyrights thereof shall constitute Powerhouse Theatrical Society sole property. This authorization and release shall be interpreted and enforced in accordance with the laws of the Province of British Columbia, Canada.

I acknowledge and represent that I am the Parent or Guardian of the above-named Consenting Person and that I have read the foregoing and fully understand the contents thereof. I hereby consent and agree to the foregoing on the above-named Consenting Person’s behalf.
